Maryland Gardens Care Center, located
in Phoenix, AZ consists of a 60 bed Skilled Nursing Unit
and a 40 bed Assisted Living Unit. This facility has worked
closely with a number of providers in the state to develop
unique programs such as behavioral and dementia care.
Through its development of specialized programs and its
unique outdoor setting, Maryland Gardens Care Center has
provided Arizona with programs that are well-renown throughout
the healthcare community. The facility is situated on

and is currently leased to Senior Management Resources
II, LLC, a Phoenix based operator of SNF’s and AL’s.
